Registration Process

Department of Minimum Standards requires that all the steps are completed BEFORE a child can begin. You may download/print the forms prior to visiting the Little Genius Club to speed up the process.

Admission Requirements

Its simple to enroll your child with Little Genius Club. Simply follow the three easy steps below. We have provided copies of the registration forms online so that you can view, print and complete them prior to arrival to speed up the registration process.

Step 1: Download the required forms

• Admission Information Sheet
• Emergency Information Card
• Authorization for Emergency Medical Care

Step 2: Gather required documentation

• Child’s Immunization Record
• Parents’ Driver’s License or State-Issued Identification
• Driver’s License or State-Issued Identification for anyone who will pick up the child

Step 3: Visit Little Genius Club

• Please bring payment for registration plus first week. We accept personal checks, cashiers checks, money orders, debit and major credit cards.
